Job Description

We are seeking a skilled Case Manager with acute care experience and a strong background in discharge planning and care coordination. The successful candidate will be responsible for facilitating the discharge process for patients within a Level II Trauma Center while collaborating with an interdisciplinary team.

Responsibilities

• Assist in the discharge planning process for patients, ensuring smooth transitions to the next level of care.
• Coordinate care among various departments, including Med-Surg, ICU, and Emergency Department.
• Conduct needs assessments and implement care plans based on patient requirements.
• Work collaboratively with social services and other healthcare professionals.
• Utilize the EPIC charting system for documentation and tracking of patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of acute care experience.
• Strong background in discharge planning and care coordination.
• Experience in various clinical settings such as Med-Surg and EDC.
• Current BLS certification and valid RN licensure.
• Experience with EPIC charting system is required.
