Monday-Friday, 8:00 AM - 4:30 PM with possible rotation of 1 weekend day per month.

Job Description

The Home Health Nurse will provide care to patients in their homes or nursing facilities, focusing on end-stage illnesses such as cancer, heart disease, lung disease, and dementia. The main responsibilities involve administering treatment, managing patient care, and coordinating with a multidisciplinary team to ensure optimal health outcomes.

Responsibilities

• Provide skilled nursing care to patients in their homes or nursing facilities.
• Manage and administer treatments for patients with terminal conditions.
• Document patient progress in the electronic medical record (EPIC).
• Collaborate with a team of healthcare professionals to provide comprehensive care.
• Conduct patient assessments and develop care plans.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health experience.
• Current BLS certification (AHA).
• Valid West Virginia nursing license.
