12-hour night shifts from 7 PM to 7 AM with every other weekend rotation.

Job Description

The Licensed Practical Nurse (LPN) will work in a long-term care setting primarily serving patients with moderate to severe Alzheimer’s and other deconditioned rehab needs. This position requires the management of care for patients in a facility with a total of 120 beds, where the nurse will perform admissions, medication administration, and coordination of care with an interdisciplinary team.

Responsibilities

• Medication administration, including narcotics that medication aides cannot pass.
• Conduct diabetic and fall assessments.
• Perform monthly assessments and coordinate care with CNAs.
• Assist with meals due to high dysphagia in Alzheimer's patients.
• Manage CNAs and work closely with the interdisciplinary team, including PT, OT, SLP, case managers, social workers, and dieticians.

Required Experience / Certifications / Licensure

• Current BLS certification is required.
• LTC experience is essential, with preferred backgrounds in roles such as DON, Charge Nurse, or House Supervisor.
• Nursing License in the State of Maine.
