Case Manager RN - Day Shift

Schedule

5x8 Hour shifts from 07:30 to 16:00, with variations possible between 07:00 and 17:00. Weekend work is required every other weekend.

Job Description

We are seeking a Case Manager RN to provide exceptional patient care and coordination. The ideal candidate will have experience in acute care settings and be proficient in using the Epic charting system.

Responsibilities

• Conduct discharge planning and needs assessment for patients.
• Coordinate with interdisciplinary teams to ensure seamless patient care.
• Develop and implement plans of care based on patient needs.
• Maintain adherence to CMS guidelines and HIPAA regulations.
• Utilize Milliman Guidelines for patient management.

Required Experience / Certifications / Licensure

• Must have a valid Alaska RN license.
• BLS certification required; ACM certification preferred.
• Minimum of 1 year of acute care experience required.
• Must have Epic experience.
