Job Description

The Skilled Home Health RN will provide essential home health care services, including cardiopulmonary assessments, basic wound care, and medication management, to patients with various medical conditions. Responsibilities will include making skilled home visits, managing care plans, and coordinating with interdisciplinary teams.

Responsibilities

• Perform skilled home visits and provide comprehensive nursing care for patients.
• Conduct assessments and develop care plans for patients with high-risk diagnoses including heart failure, COPD, and stroke.
• Administer medications and treatments as prescribed, including IV skills and wound care management.
• Utilize medical equipment and perform diagnostic testing, such as glucometer readings and lab draws.
• Educate patients and families regarding health management and disease prevention.

Required Experience / Certifications / Licensure

• Minimum of 1-year experience in skilled home care, with OASIS training.
• Valid Florida RN license or compact license.
• Current BCLS certification.
• Experience with high-risk diagnoses and general medical care.
• COVID vaccination or valid exemption; flu shot required with mandatory mask wearing.
