Job Description

We are seeking a qualified candidate with an active California license and experience in a Skilled Nursing Facility (SNF). The ideal candidate will have knowledge of the Rehab Optima (NetHealth) computer system.

Responsibilities

• Provide nursing care to SNF patients based on their individual needs.
• Utilize the Rehab Optima (NetHealth) system for patient documentation and care coordination.
• Collaborate with interdisciplinary teams to optimize patient rehabilitation outcomes.

Required Experience / Certifications / Licensure

• Active California RN or LVN license.
• Experience working in a Skilled Nursing Facility (SNF).
• Familiarity with Rehab Optima (NetHealth) computer system is preferred.
