Physical Therapist

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs and physician directions. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs to facilitate an effective return to community activities. They serve as educators for patients, families, students, coworkers, and themselves, and perform other professional duties associated with patient care and the development of therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals aimed at progressing patients towards discharge.
• Collaborate with the interdisciplinary team to address all barriers to discharge.
• Effectively manage individual, concurrent, and group treatment sessions.
• Document medical records clearly, concisely, and accurately in a timely manner.
• Participate in student programs, providing supervision, guidance, and professional evaluations.
• Actively pursue professional development through continuing education and involvement in relevant activities.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.

Required Experience / Certifications / Licensure

• BLS Certification required prior to the first shift.
• Must hold a valid, current State license.
• A minimum of 2 years of experience is required.
• LTACH experience is desired.

Schedule

This position operates on a Day 5x8 schedule.
