Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care for assigned residents. The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served, particularly adult and geriatric populations.

Responsibilities

• Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis.
• Contribute to the creation of a culturally relevant plan of resident care, involving the resident/family and healthcare team.
• Recommend revisions to the plan of care based on resident outcomes and collaborate with the interdisciplinary team.
• Maintain current knowledge and competency in nursing practice while participating in mentoring and orientation of staff.
• Provide leadership in the professional practice setting under the direction of the Registered Nurse (RN).

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS (AHA or RQI required)
• Minimum of 1 year experience as an LPN, preferably in a long-term care facility
