Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our long-term care team. The ideal candidate will provide compassionate care to elderly patients, focusing on their daily living needs while ensuring a safe and nurturing environment.

Responsibilities

• Provide high-quality nursing care to patients in a long-term care setting.
• Assist with daily activities, medication administration, and treatments.
• Collaborate with a multidisciplinary team to develop and implement patient care plans.
• Monitor and document patient progress, reporting changes to the nursing staff.
• Help maintain a clean and safe environment for both residents and staff.

Required Experience / Certifications / Licensure

• Minimum of 1 year of clinical experience required.
• Experience in long-term care preferred.
• Dementia capable care certification is preferred.
• EPIC experience within the last 2 years is non-negotiable.
• Current LPN licensure is required.
