Job Description

We are looking for a skilled Home Health Nurse with experience in home care to provide essential medical care for various patients, particularly those with cardiopulmonary needs. The ideal candidate must demonstrate expertise in OASIS documentation and possess strong IV and wound care skills.

Responsibilities

• Conduct thorough cardiopulmonary assessments.
• Provide basic wound care and manage wound vacs.
• Administer foley catheter changes and care.
• Perform staple/suture removal and glucometer use.
• Conduct lab draws via venipuncture.
• Deliver patient education and ensure safety with high-risk diagnoses such as HF, COPD, MI, Pneumonia, and Stroke.
• Participate in interdisciplinary care with Physical Therapy and Social Services as needed.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home care experience is required.
• Experience in OASIS training and home health documentation.
• Valid Florida RN license or compact license.
• Current BCLS certification.
• Must be COVID vaccinated or have an approved exemption.
• Flu shot required or declination on file; mask-wearing is mandatory while at work.
