Job Description

We are seeking a qualified Home Health Nurse with OASIS experience to provide quality care to assigned patients. This role involves assessing patient and caregiver needs, developing and communicating care plans, and managing caseloads effectively.

Responsibilities

• Utilize nursing processes to assess patient/caregiver needs and provide quality care.
• Develop realistic and evidence-based plans of care and include patients and caregivers in the process.
• Serve as Case Manager and actively manage caseloads in line with agency standards.
• Effectively complete OASIS documentation in accordance with agency and regulatory requirements.
• Implement medical plans of care and nursing interventions as needed.
• Remain current with high-tech skills like IV use, wound care, and infection control.
• Collaborate with LPNs, delegate tasks, and review documentation to meet patient needs.
• Evaluate plan of care effectiveness and update documentation timely.
• Provide patient and caregiver education on treatment options and care instructions.
• Coordinate patient care with physicians and interdisciplinary teams.
• Participate in peer review audits and provide constructive feedback.
• Schedule and facilitate case conferences to enhance communication within the team.
• Advocate for patient care issues and ensure quality care aligns with patient/family needs.

Required Experience / Certifications / Licensure

OASIS experience is required. Must possess knowledge of nursing regulations and agency policies.
