Job Description

The Acute Care Case Manager will play a vital role in the care coordination of patients within an acute care facility. The position requires current acute hospital experience and knowledge of admission criteria, care coordination, discharge planning, and the use of Interqual and Epic systems.

Responsibilities

• Perform care coordination for assigned patients.
• Conduct admission reviews and assessments.
• Develop and implement individualized care plans.
• Collaborate with interdisciplinary teams including social services.
• Ensure compliance with patient admission criteria and eligibility for benefits.
• Prepare for and manage discharge planning effectively.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care hospital experience required, with at least 6 months in a similar role.
• Knowledge of Interqual criteria and Epic charting system is essential.
• Must possess a valid California State License; pending licenses may be accepted.
• Prior experience in case management, care coordination, and discharge planning is essential.
• Travel experience in healthcare settings is required.
