Day shift, 5x8 hour schedule (08:00 - 16:30). Every other weekend required.

Job Description

The Acute Care Case Manager is responsible for managing patient caseloads with a focus on discharge planning and care coordination. This role involves assessing patient needs, developing treatment plans, and ensuring adherence to evidence-based guidelines.

Responsibilities

• Conduct assessments and determine medical necessity per evidence-based guidelines.
• Develop and execute plans of care in collaboration with interdisciplinary teams.
• Coordinate discharge planning and arrange necessary follow-up services.
• Facilitate family meetings and discussions as needed.
• Maintain compliance with CMS guidelines and HIPAA regulations.
• Document all interactions and care plans using the Epic charting system.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in acute care case management/discharge planning.
• Active California state nursing license (pending license accepted, must obtain upon start).
• Experience with InterQual guidelines preferred.
• Previous charge experience is beneficial.
• Understanding of Medicare and Medi-Cal frameworks.
