Job Description

The Ambulatory Care Nurse will support the coordination, integration, communication, implementation, and evaluation of nursing practice and clinical policies and procedures. This role involves participating in quality and performance improvement activities, collecting patient healthcare data, and collaborating with licensed providers to develop patient-centered care plans.

Responsibilities

• Perform physical examinations and diagnostic tests.
• Collect patient health histories and document in electronic health records.
• Administer medications and treatments, including IV therapy and vaccinations.
• Conduct phone and in-person triage for clinical concerns.
• Assess and monitor patient responses to treatments and procedures.
• Collaborate with licensed providers to create and adjust patient care plans.
• Engage in quality initiatives and support patient safety initiatives.
• Educate patients and families on health-related issues and treatment plans.
• Perform case-finding to identify unmet patient needs and report to providers.
• Participate in evidence-based practice and clinical inquiry initiatives.

Required Experience / Certifications / Licensure

• Minimum 2 years of experience in a nursing role.
• Current BLS certification.
• Current ACLS certification.
• Active New York RN License.
