Job Description

An experienced Licensed Practical Nurse (LPN) is needed for our Long Term Care facility. The ideal candidate will provide high-quality care to residents and support their daily needs while adhering to best practices and regulatory standards.

Responsibilities

• Deliver patient care in a Long Term Care setting.
• Administer medications and treatments as prescribed.
• Document patient assessments and care plans accurately.
• Collaborate with healthcare team members to ensure comprehensive care.
• Assist in the development and implementation of resident care plans.

Required Experience / Certifications / Licensure

• Current, unrestricted Colorado LPN license.
• Experience in Long Term Care (LTC) is required.
• MDS experience is preferred.
