Day shift: 8 am to 4:30 pm, Monday through Friday with a weekend rotation.

Job Description

We are seeking an experienced Registered Nurse (RN) to join our Home Health team in a new, modern facility. The ideal candidate will have the ability to work with minimal direction and supervision, providing essential care to patients in their homes, which may include assisted living or adult foster homes.

Responsibilities

• Provide personalized nursing care to home health patients.
• Assess patient needs and develop care plans.
• Collaborate with healthcare team members for patient well-being.
• Document patient care accurately and in a timely manner.

Required Experience / Certifications / Licensure

• Minimum of three years of nursing experience required.
• BLS certification is mandatory.
• Home health experience preferred.
• Must possess a valid driver’s license and have your own automobile for work duties.
