Part-time positions available, with shifts scheduled as needed including 2x8 or 3x8 hours, every other weekend, and holidays as needed.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, demonstrating the knowledge and skills necessary to care for adult and geriatric patients. This role requires collaboration with the Registered Nurse and other healthcare team members, adherence to policies and procedures, and a commitment to ongoing professional development.

Responsibilities

• Collects and communicates data relevant to residents' physical and psychosocial conditions to the healthcare team.
• Contributes to creating a developmentally appropriate and culturally relevant resident care plan.
• Recommends revisions to the plan of care based on evaluation of resident outcomes.
• Maintains current knowledge and competency in nursing practice.
• Participates in mentoring and orientation of new staff.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI)
• Minimum of 1 year experience as an LPN, preferably in a Long-Term Care facility
