Job Description

We are seeking an experienced RN specializing in Case Management to join our team in Portsmouth, NH. The ideal candidate will have a strong background in acute hospital case management, critical care experience, and be skilled in utilization management, care coordination, and discharge planning.

Responsibilities

• Utilization management and care coordination
• Discharge planning and revenue integrity
• Leadership and teamwork within the case management department

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in a hospital setting
• Active Compact License required at the time of submission
• BLS certification required
• Acute hospital RN Case Management experience
• Critical care experience preferred
