Day shifts of 5x8 hours, with potential times of 07:00 to 17:00. Weekends required every other weekend.

Job Description

The Case Manager RN will work in an acute hospital setting, focusing on discharge planning and care coordination for multiple patient demographics including infants, adolescents, adults, and geriatrics. Preferred candidate will possess acute care experience and familiarity with Epic charting.

Responsibilities

• Manage patient cases and coordinate discharge planning.
• Utilize Epic for patient charting and documentation.
• Collaborate with interdisciplinary teams to provide comprehensive patient care.
• Conduct needs assessments and develop care plans in accordance with CMS guidelines.

Required Experience / Certifications / Licensure

• Must have an active RN license in Alaska.
• Minimum of 2 years of experience in case management within an acute care setting.
• Certification in BLS and preferred ACM certification.
• Experience with Epic charting system is required.
