Job Description

The Case Manager RN will focus on discharge planning and care coordination for patients in an acute care setting. The position requires collaboration with interdisciplinary teams to ensure effective patient care and smooth transitions of care.

Responsibilities

• Coordinate discharge planning and care assessments.
• Ensure compliance with CMS guidelines and HIPAA regulations.
• Perform needs assessments and order durable medical equipment (DME) as necessary.
• Utilize Epic for charting and documentation.

Required Experience / Certifications / Licensure

• Minimum of 2 years of nursing experience in an acute care setting.
• Active AK RN license required.
• BLS certification required; ACM certification preferred.
• Prior experience with Epic charting system is essential.
