Job Description

The Home Health Physical Therapist will provide skilled care to patients in their homes, focusing on rehabilitation and recovery. Responsibilities include assessments, creating and implementing home exercise programs, and educating patients and families on care practices. The ideal candidate will have experience with a variety of conditions including orthopedic, neurologic, and cardiopulmonary diagnoses.

Responsibilities

• Conduct assessments and evaluations of patients' physical conditions.
• Create and implement individualized treatment plans.
• Educate patients and families on medication management and therapy exercises.
• Monitor and document patient progress towards rehabilitation goals.
• Utilize adaptive devices and equipment as necessary.
• Ensure patient safety and adherence to best practices in care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health experience required.
• OASIS training preferred.
• Valid Florida state license or compact license required.
• Current BLS certification is mandatory.
• COVID-19 vaccinated or have an exemption on file.
• Flu shot or declination required, with mandatory mask wearing while on duty.
