Schedule
Sunday - Saturday, 10 shifts variable between 7 AM - 5:30 PM, including weekend rotation with call.
Job Description
The Nurse Case Manager, Emergency Department (ED) is responsible for establishing, coordinating, and maintaining processes that increase patient throughput to the most appropriate level of care, while facilitating interdisciplinary care across the continuum for the ED. The Nurse Case Manager works closely with the patient/family, multidisciplinary team, physicians, community resources, and payers to ensure the patient's progress and level of care is appropriately determined for the emergency episode of care and the inpatient encounter.
Responsibilities
- Evaluate patients in the ED for medical necessity and care coordination needs.
- Assess potential admissions and determine appropriate admission status with advanced practice clinicians/physicians.
- Propose and implement outpatient plans for safe discharge from the ED, subject to physician approval.
- Coordinate care for patients with abuse, chronic pain, and frequent utilization issues.
- Collaborate with multidisciplinary teams to monitor progress and ensure efficient hospitalizations.
Required Experience / Certifications / Licensure
- Associates degree in Nursing.
- Valid, unrestricted Professional Nursing License to practice nursing in any state or territory of the United States.
- Current Basic Life Support (BLS) certification by the American Heart Association.
- Case Manager Certification (CCM or ACM) must be obtained within 2 years from date of hire.
- Minimum of 2 years of current care/case management experience in the Emergency Department.
- Proficient with Electronic Medical Records, Microsoft Office, and outlook.
- One year of travel experience preferred.
