Job Description
The Integrated Case Manager for Population Health is an integral member of the patient-centered care team responsible for collaborative assessment, planning, facilitation, care coordination, evaluation, and advocacy. This role is vital in meeting the comprehensive healthcare needs of individuals and families through effective communication and resource utilization.
Responsibilities
- Conduct comprehensive assessments of patients' biomedical, psychological, social, and functional needs.
- Develop personalized, patient-centered care plans.
- Engage patients and families as part of the care team through advocacy and communication.
- Utilize professional judgment and critical thinking to aid patients in overcoming barriers.
- Provide counseling related to treatment decisions and end-of-life issues.
- Coordinate seamless patient transitions between care settings.
- Advocate for appropriate service delivery within the health plan’s benefit structure.
- Collaborate with team members to co-manage complex medical and social needs.
- Evaluate patient responses to care plans and modify as needed.
- Participate in process improvement activities to reduce risks.
- Facilitate interventions in cases of abuse and neglect.
- Support departmental goals contributing to organizational success.
Required Experience / Certifications / Licensure
- Minimum 5 years of acute inpatient case management experience.
- Strong computer skills, including Epic EMR experience.
- Nursing degree or MSW required.
- BLS certification through AHA.
- Current valid Michigan RN or LMSW licensure.
- Acute discharge planning experience with recent acute inpatient case management.
