LPN - Full Time Nights with Weekend Commitment

This position is for full-time nights, specifically requiring every weekend (5x8s, 11:00 PM - 7:30 AM), with potential for holiday shifts.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care for assigned residents, promoting a healthy and safe environment while ensuring adherence to protocols. This role involves maintaining collaborative relationships with nursing staff, physicians, and ancillary staff.

Responsibilities

• Collect and assess data pertinent to the resident's physical and psychosocial condition on an ongoing basis.
• Develop, implement, and revise individualized care plans in collaboration with residents/families and the interdisciplinary team.
• Ensure the delivery of culturally relevant care, addressing all aspects of resident care.
• Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
• Participate in the mentoring and orientation of new staff members.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure
• Basic Life Support (BLS) certification (AHA or RQI specifically)
• Minimum of 1 year of experience as an LPN, preferably in a long-term care (LTC) facility
