Occupational Therapist - Home Health

Location

Area #1 encompassing Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City; AND either Area #2 or Area #3.

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift with guaranteed or non-guaranteed hours available.

Job Description

CVHCare is seeking a qualified Occupational Therapist to provide home health services. Home health experience is preferred but not required. The role includes the possibility of seeing a diverse patient population, including those who are Covid positive or suspected cases.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to promote patient recovery and quality of life.
• Document patient progress and treatment outcomes thoroughly.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Travel to various locations within assigned service areas for home visits.

Required Experience / Certifications / Licensure

• Current CA Occupational Therapy License.
• BLS Certification.
• Valid Driver's License and auto insurance.
• TB Test (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, and Physical.
