Job Description

CVHCare is seeking a qualified Physical Therapist to provide care in designated territories, including Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville. The position may require travel outside the assigned area and weekend coverage.

Responsibilities

• Conduct ongoing patient evaluations and provide appropriate therapy interventions.
• Maintain productivity of at least 27 points per week, with a minimum of 30 points required for travelers.
• Be willing to see Covid+ and Covid Person Under Investigation (PUI) patients as needed.
• Document patient progress, treatments provided, and communicate with healthcare teams.

Required Experience / Certifications / Licensure

• Current CA Physical Therapy License
• BLS certification
• Valid Driver's License
• Automobile insurance and registration
• Annual TB test
• MMR, Varicella, HepB, TDap (every 10 years), and annual Flu vaccinations
• Drug screening results and a physical examination
