Night Shift: 5x8, 40 hours per week, with weekend requirements every other weekend.

Job Description

We are seeking a dedicated Licensed Physical Therapist to provide professional rehabilitation therapy to patients in their homes. The therapist will be responsible for evaluating patients, developing individualized treatment programs, and implementing therapy in accordance with organizational policies and external standards.

Responsibilities

• Complete patient evaluations within specified timeframes, ensuring a holistic assessment of client conditions.
• Develop individualized treatment plans based on evaluation findings that include functional and measurable goals.
• Document progress notes, reassessments, and discharge summaries according to policy and procedures.
• Ensure documentation supports the need for continued skilled care at home and is submitted timely.
• Coordinate with physicians and healthcare teams for effective patient care and resource referral.
• Supervise and assess the performance of Physical Therapy Assistants (PTA) as needed.
• Educate patients and caregivers on home programs, providing comprehensive information based on their needs.
• Participate in discharge planning and address any problem areas during patient care.
• Complete scheduling requirements accurately and respond to communications in a timely manner.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in Physical Therapy; home care experience strongly preferred.
• Bachelor's Degree in Physical Therapy required; Doctorate preferred.
• Registered Physical Therapist license required.
• BLS certification must be from the American Heart Association.
• Experience with OASIS assessments and familiarity with EPIC documentation preferred.
