Job Description

The Acute Care Hospital Case Manager will be responsible for performing care coordination and discharge planning. Candidates should have experience in an acute care hospital setting to handle the specific needs of patients across various demographics, including adolescents, adults, and geriatrics.

Responsibilities

• Utilize InterQual Criteria and DRG for care assessments.
• Coordinate patient care and discharge planning effectively.
• Develop and implement individualized plans of care in collaboration with interdisciplinary teams.
• Assess patient needs, including those with complex social determinants of health.
• Participate in family meetings and treatment plan development.
• Provide education on drug/alcohol treatment options and resources for patients in crisis.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care hospital case management experience.
• CA state license required; pending licenses are accepted if licensed upon start date.
• Experience with care coordination and discharge planning within an acute care environment is a must.
• Proficiency with Epic charting system.
• Ability to work independently and arrange necessary services for patients.
