Job Description

Caledonia Home Health Care & Hospice is seeking a dedicated Physical Therapist to provide in-home therapy services. The successful candidate will focus on maximizing the independence and safety of our home care and hospice patients. Must work independently and as part of a collaborative multidisciplinary team.

Responsibilities

• Provide in-home physical therapy services to patients.
• Develop individualized treatment plans aimed at maximizing independence.
• Collaborate with a multidisciplinary team to deliver holistic patient care.
• Maintain accurate and up-to-date patient records and documentation.

Required Experience / Certifications / Licensure

• VT licensure as a Physical Therapist.
• Valid driver's license and reliable vehicle.
• Experience in a home care setting is preferred.
