Schedule

Part time Day and Evening shifts, scheduled as needed by the facility (generally 2x8s/3x8s) with weekend and holiday shifts required.

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. This includes assessing data reflective of the patient's status and interpreting the information needed to identify each patient's requirements relative to his or her age-specific needs. The LPN provides direct and indirect skilled nursing care for assigned residents while maintaining a collaborative relationship with other nursing staff and healthcare teams.

Responsibilities

• Collects data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicates findings to the RN and other members of the Interdisciplinary Team.
• Contributes to the creation of developmentally age-appropriate and culturally relevant care plans.
• Recommends revisions to the plan of care after evaluating resident outcomes.
• Collaborates with residents, families, and healthcare team members to ensure all aspects of care are addressed.
• Maintains current knowledge and competency in Nursing Practice and participates in staff mentoring/orientation.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (AHA or RQI specific)
• 1 year of experience serving in an LPN capacity at a Long-Term Care facility
