Job Description

We are seeking a dedicated Home Health RN with at least 2 years of experience, including expertise in OASIS and wound/vac care, to provide high-quality nursing care in the home health setting.

Responsibilities

• Perform comprehensive assessments and develop personalized care plans.
• Deliver wound care management, including VAC therapy.
• Complete OASIS documentation accurately and timely.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on health management and care techniques.

Required Experience / Certifications / Licensure

• Registered Nurse (RN) license in Pennsylvania.
• Minimum of 2 years of nursing experience, preferably in home health.
• Proficiency in OASIS documentation.
• Experience with wound care and VAC therapy.
