Schedule

Day shift, 5x8-hour schedule (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

The Home Health RN will provide skilled nursing care to patients in their homes, ensuring that their medical and emotional needs are met. The nurse may visit up to 6 patients per day, maintaining a productivity standard of 30 points.

Responsibilities

• Conducting admission assessments, recertifications, and resumption of care.
• Performing routine visits and start of care assessments.
• Providing wound care and medication management.
• Utilizing infection control precautions.
• Implementing pain assessment and management strategies.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• BLS certification is required.
• OASIS experience is preferred.
