Patient Management RN - Days Shift - 13 Weeks Contract

Schedule

This position follows a DAYS shift from 8:00 AM to 4:30 PM, Monday through Friday, totaling 40 hours per week. There are no weekend or holiday shifts required.

Job Description

The Patient Management RN will focus on acute care discharge planning. The role requires familiarity with the discharge planning and Utilization Review (UR) processes, as well as knowledge of community agencies to aid in patient transition after discharge.

Required Experience / Certifications / Licensure

• Minimum of 1 year of Medical/Surgical and/or Community Nursing experience.
• Prior experience in acute care discharge planning is essential.
• Familiarity with the discharge planning/UR process and community agencies.
• Experience with EPIC software is required.
• For non-U.S. candidates, 2 years of experience is required.
• Active NY RN license is a must.
• AHA Basic Life Support (BLS) certification is required.
