Part Time Nights (2x8s/3x8s) with Every Other Weekend and Holiday required. Must be able to start on a Tuesday or Thursday.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care to assigned residents in accordance with their individualized care plans. This position follows defined policies and procedures and works under the supervision of a Registered Nurse. The LPN must possess the knowledge and skills necessary to provide age-appropriate care to adult and geriatric patients.

Responsibilities

• Collect and communicate data relevant to residents' physical and psychosocial conditions to the RN and the interdisciplinary team.
• Contribute to the development of culturally relevant care plans in collaboration with residents, families, and healthcare team members.
• Recommend revisions to plans of care based on evaluations of resident outcomes and ensure all aspects of care are addressed.
• Maintain current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.
• Exercise leadership and demonstrate knowledge and competency in the professional practice settings.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS Certification (AHA or RQI Only)
• Minimum of 1 year experience as an LPN in a Long-Term Care Facility
